As a part of our Masonic Lodge approval process, you must successfully complete a background check. In order to make this process quick and easy, the Grand Lodge, F. & A. M. Of Ohio requires that you complete the following request. You will need to provide your full name, date of birth and social security number. By providing this information electronically it can be safely and securely transmitted to our background check vendor instantly. This enhanced process can remove days of waiting from the overall process. The cost of the report will be $13.95. If you are a resident of New York then an additional $98.00 fee will be requested. If you DO NOT Receive your Report within 5 BUSINESS DAYS via eMail you must SUBMIT A SUPPORT TICKET Please check your email SPAM folder before contacting us. If your criminal report has "Hits" it will be forwarded to the QA Dept. for further investigation and verification of records. This will delay your report from 1 to 3 business days. There will be a section requesting your resume, skip this step by clicking Save and Continue ***IMPORTANT*** To recieve a copy of your background report via email, you must select the option under Summary and Review labeled "Please check this box if you would like to receive a copy of a consumer report." If you do not select this option then you will not recieve a copy.
